Understanding Your Health Record
Each time you receive services from NCA Home Health Care, Inc., a record of your visit and care is created. Typically, this record contains your symptoms, examination and test results, diagnoses, treatment plan, and billing details. This information, often referred to as your health or medical record, serves as:
- A basis for planning your care and treatment.
- A means of communication among the health professionals who contribute to your care.
- A legal document describing the care you received.
- A tool by which a third-party payer can verify that services billed were actually provided.
Our Responsibilities Under HIPAA
Under HIPAA, NCA Home Health Care, Inc. is required to:
- Maintain the privacy and security of your Protected Health Information (PHI).
- Provide you with a notice describing our duties and privacy practices regarding PHI.
- Notify you promptly if a breach occurs that may have compromised the privacy or security of your unsecured PHI.
- Follow the terms of the Notice of Privacy Practices currently in effect.

Please note that standard email and web submit forms may not be secure methods of communication. Do not submit sensitive medical or personal information through unencrypted web forms unless explicitly designated as secure.
